# Creating an Action Tracker

The Monitoring Action Tracker (MAT) records a review, such as an internal audit, an external audit or a regulator visit, and tracks the findings raised by it until each one is resolved. This article shows how to create the tracker record. Findings are added afterwards.

**Note:** **Applies to:** users with a Monitoring Action Tracker role that can create records (for example `tracker_admin` or `tracker_record_owner`), or system administrators.

## Opening the Monitoring Action Tracker

1. Select **Monitoring Action Tracker** in the left menu. The dashboard shows the trackers by Status, Source and Overall Assessment Grade, with all trackers listed below.

1. Click the **+** icon at the top right to start a new tracker.

## Filling in the Tracker Details

1. Enter a **Summary** of the review being tracked, for example "Annual internal audit of supplier onboarding". This is the only required field.
2. Check the **Status**. New trackers start as **Open**.
3. Select the **Source**: the team, entity or body that raised the review, such as Internal Audit, External Audit or a Regulator.
4. Select the **Overall Assessment Grade** given by the review. Most trackers start at a grade such as **Improvement Needed** and are updated as findings are closed.
5. Pick the **Final Report Date** with the calendar icon.
6. Check the **Owners**. The person creating the tracker is added automatically; add or remove owners as needed.

**Note:** The **Reference** is numbered automatically when you save. The Status, Source and Overall Assessment Grade options are managed by an administrator under the Monitoring Action Tracker menu (Status, Source, Rating and Overall Assessment Grade).

## Saving the Tracker

1. Click **Save** at the top right.

The tracker now appears in the All Trackers grid. Open it to add findings.
